provide intensive transitional services to people at risk of being permanently institutionalized navigate and facilitate the resources available within the community to ensure independent living funded by the Health Plan of San Mateo must be Medicare/Medi-Cal enrollees

5 The goal is independent living and well-being
Health Plan of San Mateo will assess candidate for 3 months to see if ready to transition back into the community Send referral to IOA who will then connect the individual to all possible resources available to assist with independent living IOA case manager will follow for six months to ensure individuals are living in their homes safely The goal is independent living and well-being (
