Imagine What if we created a local system of health, human, and social services that cannot fail? What if people got access to services and supports on their own terms? What if we treated every person that comes through our doors like they’re entering a 5-star hotel? What if we broke down our silos and worked together whenever possible? What if we measured success by a person’s whole quality of life? What if we stopped a system that thrives on illness and make it thrive on wellness? What if we repaired the social fabric of our communities?

Integration Platform The Imagine Dutchess Platform is a web-based platform that will unite the county’s behavioral health, primary health, and health & human services community providers into a single virtual location so that individuals can access the services and information that they need in one place, and so that providers can work together on common goals. It has become imperative and obvious for systemic integration to work efficiently and effectively, all provider partners have to be in agreement to be immediately responsive to the needs of our community in an improved collaborative way The community also realized that integration cannot only happen in a virtual reality world. We had to create examples. And we did.

A Paradigm Shift Evolving the system
Whole Health Oversight, where physical and behavioral health, as well as social concerns are addressed immediately Person-Centered/Peer led Team Approach Diversionary from the emergency department and criminal justice system

Project goals Improve the integration of service delivery throughout the Dutchess County community Provide alternatives to criminal justice, emergency department visits and linkage with “bridging” to facilitate shorter inpatient mental health admissions and/or jail Quick drop off and gateway to wellness Provide CIT training to all police, probation, jail based CO’s in Dutchess County

The Partners Mid-Hudson Regional Hospital, Mid-Hudson Addiction Recovery Centers, PEOPLe, Inc., and Astor Services for Children & Families The Center’s partners will bring specialized expertise to the services provided, addressing the physical health care needs, as well as substance use and mental health. Provide peer support and bridging services to guests, as needed The intention is to improve access to services, quality of life and better engagement for long-term recovery goals

Stabilization CEnter Participation in services at the Center are 100% voluntary Services across the life span Seamless transitions to partner agencies Bridging to ensure engagement and follow-through with services

Stabilization Center People experiencing a behavioral health crisis will be welcome to walk into the Center 24/7/365 First responders, including law enforcement, and families will have a warm hand-off to mental health caregivers to continue the work of triage and stabilization Easy and open access to services and supports when people need them Trauma-Informed Approaches and an environment that establishes safety and security---physically and psychologically

What happens at the Stabilization Center?
Medical Triage Motivational Assessments, addressing mental health and substance use disorders Screening, Brief Intervention, and Referral to Treatment Sobering beds, initiating detox (8 beds) 23-hour psychiatric observation beds (5 beds) Family and Adult Respite spaces Transition planning to appropriate levels of service is continuous

What Happens at the Stabilization Center?
At the Crisis Stabilization and Wellness Center, guests will have the opportunity to engage with other community partners/services: Dept. of Community & Family Services (Social Services) Probation Care Coordination Domestic Violence Services Housing Other Related Providers/Services

Staffing Dutchess County is the umbrella agency for the Stabilization Center Dedicated staffing from the Department of Behavioral & Community Health’s Division of Mental Hygiene will address the supervisory needs of the Center HELPLINE and the Mobile Crisis Intervention & Prevention Team will maintain a 24-hour presence
