2. To Register:
Navigate to secure.medproctor.com
Click New Users - Register

3. Complete registration by:
Entering and confirming your University address
Read the License and Policies and click the box
Click Register

4. Begin completing all workflow steps, starting with Personal Contact Information under the Profile tab.

5. After completing all required fields under Personal Contact Information, click Continue to move to the next section.

6. The next section under Profile is Emergency Contact Information. Click Continue when completed to move to the next section.

7. Entering Term is the next section
Entering Term is the next section. It is very important to select the correct Entering Term. When completed, click Continue to move to the next section.

8. Agreements is the next section
Agreements is the next section. Read the EULA and electronically sign the agreement by entering your full name (same full name you entered in the Personal Contact Information section). Click Agree & Continue to move to the next section.

9. Medical ID - Personal Basics is the first section under the Medical tab. Fill in all required fields and click Continue to move to the next section.
Note the Medical, Insurance, and Document tabs populate after signing the EULA.

10. Allergies is the next section under the Medical tab
Allergies is the next section under the Medical tab. Check all that apply and click Continue at the bottom of the list.

11. Medical History is the next section under the Medical tab
Medical History is the next section under the Medical tab. Click Add Conditions to add a new previous medical condition. After adding all conditions, click Continue to move to the next section.

12. Medications is the next section under the Medical tab
Medications is the next section under the Medical tab. Click Add a Medication to add any medications you are currently taking. When all medications have been added, click Continue to move to the next section.

13. The Insurance Questionnaire is the only section under the Insurance tab.
Select Yes or No and click Continue to move to the section.

14. Choose one of the two options:
Pay $10 for a Med+Proctor Pro account, or
opt-out of a pro account and continue for free.

15. Carefully read these detailed instructions before downloading your forms.

16. Download the Med+Proctor Immunization Certificate by clicking this link. Print your form once it has downloaded to your computer.

17. Note the immunizations Required by ETSU that must be included on the second page of this form.
The first page of the form is the Instructions page. Carefully read these instructions.

18. The second page of this form is your actual Immunization Certificate
The second page of this form is your actual Immunization Certificate. Get this page filled out and signed by your Doctor. Take a picture or scan this page back to your computer and save it as a .jpeg/.jpg or .pdf.
Note all green fields on this form are Required and must be filled in.

19. The third page of this form is the Hepatitis B and Meningococcal Meningitis Immunization Health History Form. Carefully read this entire page and sign where prompted. Take a picture or scan this page back to your computer and save it as a .jpeg/.jpg or .pdf.
Note if you are under the age of 18, a parent/guardian must sign this form as well.

20. Note the list of Required and Optional Documents
Note the list of Required and Optional Documents. You must upload all documents labeled Required.
To upload page 2 and 3 of your Immunization Certificate:
Choose the Document Type for the page you are uploading from the dropdown list
Click Choose File to find the page you are trying to upload and add it
Click Continue to upload

21. The system will recognize the number forms you upload
The system will recognize the number forms you upload. In order to see the forms you uploaded it is not necessary to upgrade your account.
