1 Practice Based Assessments in Primary Care

2 Aim To illustrate the use of Case based Discussions (CbD) for the assessment of clinical advanced practice of pharmacists in primary care or in a patient facing role Reminder of the overall aim of the session

3 Sharing Our Experience
We are from CPPE and are responsible for delivering GPPTP Practice based assessments are part of the pathway including CbD We have experience of using CbD with 490 pharmacists on the pathway CbD is also part of RPS Faculty assessment

4 Learning Objectives Define the meaning of a CbD
Explain the role of CbDs in RPS Faculty credentialing of advanced practice Summarise the structure and process for a CbD Describe the CbD advanced practice assessment criteria List questions that could be asked during a CbD State how to give developmental feedback to a peer to support their professional development

5 What is a CbD? Discussion of an individual’s practice with a peer, colleague or tutor Complex case, intervention, project or initiative Can be used for many different situations e.g. designing a study day, organising and delivering a study day being an expert speaker at a conference or event poster or presentation at a conference delivery of a complex project or service improvement etc. Practice based assessment – extending beyond what they know Individual to explain why they acted as they did and demonstrate what they have been able to do in practice

6 PBA Assessment Assess practice against criteria and competencies Provide feedback Encourage reflection to support the individual in identifying strengths and areas for development Support the individual with a PDP to develop their practice further

9 Structure of a CbD Pharmacist presents their case
Peer or tutor [Assessor]: asks questions for clarification and probes for further details provides developmental feedback completes the CbD form informs pharmacist of assessment outcome

10 CbD in General Practice Pilot
The case presented must be clinical because the NHSE pilot is about developing the clinical patient facing role CbDs are conducted within learning sets Pharmacists present their case in turn to their peers and an Assessor – the clinical mentor – who facilitates the learning sets Clinical Mentors undertake CbD Assessor training, developed by CPPE

In your groups, discuss what questions you need to ask to clarify any details probe for additional information

12 Activity 2 (18mins) Refer to CbD case – Questions asked handout
Watch Video 1 Compare your list of questions with those asked by the Assessor Share any differences during group discussion

Write down the feedback you would provide for this pharmacist using the Developmental Feedback model to structure your feedback

15 Feedback - How are we doing?
Pharmacist Clinical Mentor “[I] dealt with a patient differently the next day as a direct result of listening to someone else's case.” “…able to share resources which were found useful or certain phrases to explain to patients.” “It was actually a really good relationship building session doing the feedback session.” “I had been concerned if my own clinical skills would be up to scratch. But I found this was not the case.”
