Background Because of an established working relationship with the ‘Little Havana Centers’, BAROMA Healthcare has been contracted to build the foundation of and launch an HMO, including building the provider network. The providers you will be contracting are all located in Areas 1 and 2, in the Panhandle of Florida. The two principle population centers in the region are Pensacola and Tallahassee.

Description All Florida Healthcare Services is a not for profit start-up health plan that is pending approval as a Medicaid Health Plan in Florida. As part of Little Havana Activities & Nutritional Centers, AFHS has been servicing the elderly communities of South Florida for over 40 years; feeding the hungry, helping the needy get to their medical appointments, and providing services for the elderly in the best environment for them – their homes. Description AFHS has made a commitment to serve those in their community who are socially isolated and economically disadvantaged. By extending their services to all age ranges and expanding into an HMO, AFHS will have the ability to continue to do so after the enactment of Florida HB 7107— Medicaid Managed Care.

How You will call targeted providers and follow up on the status of the Agreements that have been mailed, ed, and faxed. Request that the signed agreement, application and the requested credentials be mailed within 30 days of first contact. You will call each provider twice a week, minimum, until the Agreement has been either mailed in completed or declined.

How Each have you have received a unique excel spreadsheet titled ‘Progress Log’.

How You are expected to make a total of 60 calls a day and update your progress log accordingly.

How Please make sure to verify all Demographics on the spreadsheet and make additions or corrections where applicable. Missing or corrected information should be obtained (office manager, , etc…) and entered in your progress log as described in the presentation (Do not change - always add new). You will need to verify if the provider has additional locations. If there are additional locations you will need to insert a row under the initial provider and enter the additional demographics on the spreadsheet in the same manner as when adding or correcting information. Never change original or previously entered information. Always insert a new line below previous and enter new data.
