Data Quality Case ascertainment (Data Completeness) & Data Accreditation The Trauma Audit & Research Network (TARN)
2
Case ascertainment % (Quantity)
Measure of Expected v Submitted number of cases Expected no. of cases denominator derived from 2015/6 HIPE Data HIPE: Hospital In-patient Enquiry HIPE data: ICD10 codes assigned by Trust previous year: Admissions only HPO (Healthcare Pricing Office) apply TARN Inclusion criteria to data: using TARN SQL script Expected no. of cases (denominator) derived
3
Inclusion Criteria 1) All Trauma patients who fulfill the following criteria:- 2) Admission > 3 days or Admission to an intensive care area or Transferred out for continuing care or Transferred in for continuing care or Died 3) And whose injuries are TARN eligible
4
Inclusion Criteria: common inclusions/exclusions
Include (in isolation) Exclude (in isolation) Brain injury or skull fracture Skin injuries Unstable facial fracture Stable facial fracture Spinal fracture &/or cord injury Spinal strains Internal Thoracic or Abdominal injury Closed unilateral upper Limb or lower leg fractures Open upper limb or lower leg fracture Hip fracture 65+ Femur or Pelvic fractures (except Hip fracture 65+) Injuries to digits Crush or traumatic Amputation 1st or 2nd degree Burns <10% TBSA Asphyxia or Drowning Hypothermia Blood loss >20% Vascular transection Burns >10% TBSA or 3rd degree Inhalation
5
Measure of Expected v Submitted number of cases
Case ascertainment % Measure of Expected v Submitted number of cases Expected no. of cases (denominator) compared to no. submitted cases per annum (numerator) Case ascertainment % calculated for each hospital Shown in Clinical reports & Dashboards 70% target set by NOCA for 2016 Used to validate results
6
Validating the HIPE denominator
Use TARN SQL script to identify list of potential TARN cases Coordinators to note any ineligible cases & reason. Feedback to NOCA, using spreadsheet ed by them September 17. Spreadsheet contains dropdown list of ineligible reasons & ‘other’ option, similar to example below. List of ineligible reasons based on validation exercises completed by English Trusts & TARN. NOCA to send spreadsheet to HPO to adjust expected no. of cases denominator. Adjusted denominator passed onto TARN.
7
Data Accreditation % (Quality)
Measure of frequency of CORE data field completion CURRENT ACCREDITATION FIELDS Glasgow Coma Score or Intubation/ventilation (Recorded Pre hospital, ED or first GCS – All cases) Incident or 999 Date/time Arrival time Transfer reason, previous/next hospital, request date CT time Operation time, grade, speciality of surgeon, grade of anaesthetist ED doctors: time, grade & speciality Injury detail – proportion of NFS codes *Pre-existing conditions **Pupil reactivity for patients with AIS 3+ (Serious) head injuries (Recorded in any location – including 1st hospital) * Latest Probability of Survival (Ps)17 model ‘Other’ and ‘Not Known’ detrimental to Data Accreditation ** Future Probability of Survival model
8
Data Accreditation % Shown in: Clinical report Dashboards
Also used to validate published results 95%+ target
