Treatment of breast ductal carcinoma in situ (DCIS) as a rare entity in an unscreened population in a developing country Mariam Baig, Sadaf Batool, Sana Amir Akbar, Mohammad Zulqarnain Chaudhry, Amina Iqbal Khan, Muhammad Asad Parvaiz Department of Breast Surgery, Shaukat Khanum Memorial Cancer Hospital & Research Center, Pakistan INTRODUCTION The incidence of ductal carcinoma in situ (DCIS) in breast has been reported very low in Pakistan due to absence of population based screening programme. OBJECTIVE Evaluate the surgical treatment options for DCIS, in a developing country so that best surgical option can be proposed for future management. RESULTS Median Age: 48 years Breast conserving surgery (BCS): 42% Mastectomy: 58% Median tumour size: 12.5mm in Mastectomy group and 5mm in BCS group Invasive component: 0.06% BCS group and 0.14% mastectomy group. High nuclear grade DCIS: BCS group 0.32%) 0.4% in mastectomy group Overall Survival: 96% METHODS Study Period: January 2000 – December 2011 Diagnosed on core or excisional biopsy Evaluated for tumour size, resection margins, nodal disease, receptor status and nuclear grade Disease free survival and overall survival measured Breast Cancer Patients: 8269 DCIS Presented at SKMCH & RC: 94 DCIS Treated at SKMCH & RC: 73 CONCLUSION DCIS is mainly identified in symptomatic patients Relatively smaller size DCIS even with high nuclear grade, can be successfully treated with BCS.
