Transabdominal gray-scale ultrasound image of the pelvis in this 32-year-old patient who presented with abdominal pain, positive urine β-hCG, and history of cesarean section 2 years prior to presentation show the rare case of an ectopic pregnancy in a cesarean scar. (A) There is an eccentric gestational sac within anterior myometrium in the lower uterine segment at the site of prior C-section scar (dashed arrow). An empty uterine cavity (arrows) and cervix (arrowhead) are seen. (B) Sagittal transvaginal ultrasound image of the pelvis for the same patient 4 weeks later shows a viable ectopic pregnancy at the site of prior C-section scar (dashed arrow) with an empty uterine cavity filled with blood (arrows). An empty cervix is seen again. (C) Transabdominal ultrasound image shows the eccentric anterior lower uterine gestational sac (white arrow) with thin myometrium (black arrows) between the sac and bladder (asterisk) compatible with scar ectopic pregnancy.
