Start discussing termination now; When will be your last day with a student? Re-frame as a ‘Transition’, a new beginning Review successes and accomplishments Identify continued supports and resources Allow the student the opportunity to discuss feelings about ending work together Consider a termination activity

19 Termination Progress Note
Remember to write a final progress note when your work with a student ends: Case closed Case transferred Student moved End of school year If the case is transferred to you, write a note under the previous FYAPC closing note establishing that you now have the case

20 Ending and Receiving Progress Note Examples

Contacted FYAPC at ABC school to notify of the transfer, update on case information and transfer CAA. R. Coates- Campbell FYAPC met with student for final termination activity. Reviewed with student the goals set at the beginning of the year and the progress and successes the student has made since that time. Identified the student’s network of support and gave resources: names and contact information of additional school personal and community counselors to contact if needed. Student engaged in termination activity and was able to discuss positive feelings about the transition. R. Coates-Campbell
