1. Tryout Checklist
Completing all of the required steps below will streamline the tryout process with Extreme. With the exception of the NTR online registration process, all of the forms below are available to download and print on our website at Extremevbc.com.
Register your child with NTR using this link: NTR Volleyball and select NW Extreme VBC as your club when prompted. Bring a copy of your registration card to the tryout
Register for your age group tryout on Extreme website (early discount available!)
Bring a copy of your daughter’s Birth Certificate to the tryout
Bring the NTR Medical Release form (notary present at tryouts)
Bring the completed Credit Card Authorization Form. This form is required even if you are paying dues with cash or check throughout the season
Bring the completed Extreme Liability Waiver & Public Information Release
Bring the completed Player Information Sheet with jersey number preferences
Bring a check book or credit card for merchandise purchase and team deposit
We will complete the NTR Player Contract at the tryout when your deposit is received
