1 Activity and Performance Report July – September 2016

2 Four Objectives Developing our locality arrangements, to ensure that our services are responsive and connected to local communities Working with staff, stakeholders and service users to develop integrated delivery arrangements which are capable of improving the experience and outcomes of the people we serve Deploying our resources transparently, making sound investment decisions and managing a very fragile budget Implementing our strategic plan, to ensure that our services change and adapt in response to population need

3 Dental Services Negotiations to secure an independent dental practice in Stornoway, which are anticipated to come to fruition towards the end of 2016; The ongoing development of an options appraisal for the review of dental provision in the Uists. This has involved a series of discussions with the Locality Planning Group; The management of short-term pressures in Stornoway as a result of staff retiring / leaving the service Performance: Short-term issues with waiting times in Lewis

4 Primary Care Appointment of a Cluster Quality Lead (associate Medical Director) and identification of Practice Quality Leads underway – cluster work designed to improve patient quality across partnership; A partnership development day was held to review Out of Hours system, to support self-evaluation and service redesign. Consideration of national recommendations as part of programme of change; Performance The management of short-term pressures in Out of Hours continues to be a challenge. Emregent risks around CUCN recruitment need to be managed.

5 Mental Health Ongoing review of mental health service, based on principles of community based care and recovery focus. Public engagement events held, and learning from other partnerships undertaken; Scoping work undertaken to support the discharge of patients being cared for in the Clisham ward of WI Hospital; Development of a psychology service on the back of national funding;

6 Partnership Services A series of discussions around service development have been undertaken with external partners, including HC-One (Blar Buidhe) and Advocacy Western Isles Work is ongoing in respect of the renewal of long-term care capacity in Lewis, which has included engagement with both Lewis Locality Planning Groups; Establishment of Lead AHP role in progress Performance: Recruitment and retention issues being addressed in assessment and care management

7 Locality Services Development of service proposals underway to bring community nursing and social care together. This has included participation on national test site events, with our contribution focused on reform in the Uists; Implementation of home care redesign in Stornoway and Broadbay has been successful; Ardseileach core and cluster opened to service users, providing high quality care and support to adults with learning disabilities. Performance: Satisfaction survey of home care clients in Lewis and Uists produced a 93% satisfaction rate with the overall service provided

8 Delegated Hospital Services
Intense challenges for all three hospitals around delayed discharge this summer; Development of delayed discharge policy and action plan to address challenges; Redesign of Uist and Barra Hospital; Development of new St Brendan’s campus in Castlebay Performance: Major issues around delayed discharge and emergency admissions
