4 Why Coordinated Care? Team-based coordinated care is integrated across all patient visits. Patient health and wellness is the vision and goal of providers. Patients choose from a select number of quality regional hospitals, health facilities, physicians and other medical professionals. Through shared information, each patient’s health is coordinated by the entire team, ensuring patient-centric care. Coordinated care makes it easier for members and insurers to get the best care at the best price. THN is powered by three of the nation’s leading health systems– Ascension Health, Trinity Health, and University of Michigan* Health Systems. * Only select specialty services provided by University of Michigan Health System qualify for the Tier 1 benefit level; all of these services require prior authorization.

Convenient: 75% of Michigan residents are within 20 minutes of a THN provider. Quality: 28 of Michigan’s highest ranked hospitals including Ascension Health and Trinity Health Systems. 6 SafeGuard THN: Tiered Benefits
Members enjoy the best discounts and lowest out-of-pocket costs within Tier One– while receiving comprehensive, coordinated care from acclaimed THN providers and facilities within a patient-centered medical home (PCMH) care delivery model. When services are needed from a non-THN provider within Michigan, members receive Tier Two benefits when serviced by a Cofinity provider or facility. Tier Three benefits apply for all non-THN or non-Cofinity providers within Michigan and nationwide.

7 What Is a Patient Centered Medical Home?
A patient-centered medical home (PCMH) is a way to care for patients so they feel “at home” with their care. PCMH changes the emphasis from quantity to quality, and fosters a care environment based on information sharing among chosen clinicians. PCMH does not require provider elections or assignments The physician generally seen for primary care becomes the patient’s medical information coordinator and advocate Patients remain free to see other providers based on scheduling reasons or reasons of preference The relationship between patient and primary care physician is strengthened Providers are compensated for the extra effort required to deliver coordinated care THN is comprised of hundreds of physician practices designated as PCMH that act as a patient’s first point of contact

Auto Accident Coverage* With this option, medical coverage becomes primary for auto-accident related claims 24-Hour Work-Related Coverage** Covers claims for work-related illness and injuries for those not covered under group workers comp policy * Available in Michigan only ** Subject to underwriting approval
