Why Collaborative Care?
Failure to address psychiatric and substance use challenges in individuals with other medical problems leads to unnecessary costs through avoidable use of emergency rooms and medical/psychiatric acute care Separating psychiatric care from primary care/specialty services creates additional stigma and reduces likelihood of successful outcomes An opportunity for mental healthcare professionals to train as a team in a contemporary, evidence-based model of integrated care

The DellMed Collaborative Care Model
An Integral Care psychiatric social worker is assigned to the clinic team The Department of Psychiatry provides a consulting psychiatrist Together, the social worker and psychiatrist help the clinic team develop person-centered treatment plans that broadly address medical, psychological and social needs
