Wrapping for Success: Putting the PDSA Cycle in Motion
Team Activity Introduction: Making improvements requires change, but change can seem threatening or overwhelming. The Plan-Do-Study-Act cycle, or “PDSA,” is a model used to test ideas or changes that you think will result in improvement or solve a problem in your healthcare facility. The PDSA cycle breaks projects down to manageable size and tests each small part to make sure your plan for change is successful, with no wasted effort. We use PDSA cycles everyday, we just don’t realize it.

The PDSA Cycle for Learning and Improvement
Act Plan What changes will you make? Next cycle? Objective (why) Plan to carry out the cycle (who, what, where, when) Plan for collection of data (who, what, where, when) Study Do Step 1: Plan. Identify the change you think will result in improvement. Establish the scope for the change to be introduced. Identify the prediction (why). Then, plan the test of change (who, what, where, when). Establish how you are going to collect information about the impact of the change and how you will know whether the change made has “worked” or not. Create a plan for data collection (who, what, where, when). Step 2: Do. Carry out the plan. Document problems and unexpected complications. Step 3: Study. Review and reflect on the impact of the change. Analyze the data and ask, “Has there been an improvement?” “Did your expectations match the reality of what happened?” and “What could have been done differently?” Step 4: Act. Use your data to inform the next step. Is the plan ready to be adopted? Should it be adapted or modified in any way? If needed, adjust the plan to address what didn’t work. Return to Step 1 and repeat the cycle. Analyze data Compare data to predictions Summarize lessons learned Carry out the plan Document problems and unexpected complications 2
