Problem Statement Current documentation in the hospital by attendings, advanced practice providers, and residents is sub optimal for purposes of provision of patient care, billing and coding, and utilization review.

Standardize consistency and improve quality of information provided in Discharge Summaries Meetings to date: 10/14/15 (Fall retreat) 11/05/16 03/17/16 05/10/16

What has been accomplished
Review of the literature on elements of a good discharge summary. Review the VA template for discharge summaries Surveyed UNM PCP’s Agree on D/C Summary Essentials Elements Education of UNM Hospitalists/Residents Retrospective review of D/C Summary compliance with Essential Elements

PCP Survey Design: 95 responses (54% response rate)
8 item survey sent to UNM Primary Care Providers in February 98 Family Medicine and 79 Internal Medicine Included residents 95 responses (54% response rate)
