… equality of educational opportunity for all students. SDS is the designated office that obtains and files disability-related documents, certifies eligibility for services, determines reasonable accommodations, and develops plans for the provision of accommodations for students.

Students seeking academic accommodations for specific learning disabilities, medical disabilities, psychological disabilities or attention deficit disorders, are required to contact the Office of Student Affairs. The initial step in requesting accommodations is to complete the Disability Notification and Accommodation Request Form. It is the responsibility of individuals with disabilities to inform us of their needs, and to provide documentation of their disability.
