Questionnaire sent out to staff on SEAU 7 risk factor categories
Each divided into 3 subcategories Extent on ability to work effectively is assessed Results assessed with MS Excel spreadsheet Collected data and comments totalled Most common hassles identified Extent of each effect on ability to work effectively is assessed

Questionnaire This is an example of one of the basic risk factors staff were asked questions on. A rating of 1 indicated that this was not an issue for them while working on SEAU, but a rating of 5 indicated a significant problem. There was also plenty of opportunity for staff to provide more specific answers to any of the questions asked. These are particularly useful in identifying areas for improvement.
