3 Community Health Solicitation
Community Health Program 3-4 projects $3 million in funds Minimum of $500,000; maximum of $1,000,000 Up to 3-year projects Projects run December 2017 – December 2020 Health projects and services designed to reduce the risks associated with asthma and other and/or cardiopulmonary ailments. These projects may include services such as screening and diagnosis, case management, education, training community health workers or medical personnel, and outreach.

Health projects/services designed to reduce the risks associated with asthma & other respiratory &/or cardiopulmonary ailments Screening & diagnosis, case management, education, training staff, &/or outreach Applicants Serve children, pregnant women, senior citizens, the chronically ill, people with respiratory/ cardiopulmonary illnesses AND a Government agency, 501(c)3 nonprofit OR licensed health care provider Geography Priority Zone & Eligibility Zone

5 ELIGIBILITY ZONES Most of the funding will be directed to the priority zone you see here. It’s bounded roughly by the port boundaries to the south, Rosecrans Avenue to the north, Alameda Street to the west, Long Beach Blvd to the east jogging up into Paramount. Projects in this priority zone will receive a higher ranking. We also have an eligibility zone, which incorporates the city of Long Beach boundaries plus areas in Wilmington, Carson, and Paramount. At minimum, all projects must take place in this zone.

6 PROJECT OBJECTIVES Primary Objectives (Must include ONE)
Projects must achieve THREE objectives Primary Objectives (Must include ONE) Reduce number of emergency department visits and hospitalizations Decrease medical costs Reduce number of missed school and work days Increase hours of training of medical personnel and community health workers

7 PROJECT OBJECTIVES Secondary Objectives (Must include TWO)
Decrease symptom severity Increase number of symptom-free days Reduce use of rescue medication Reduce use of long-term controller medication Reduce use of quick-relief medication 2 or fewer times per week Increase % of clients with well-controlled asthma per Asthma Control Test score Reduce number of home environmental hazards such as mold, pests, and improperly opening/closing windows

10 EVALUATION CRITERIA Check all that apply Criteria
1. Application Information The applicant has provided all of the information requested under the section “applicant information” such as Name of Lead Organization, Tax ID Number, etc. 2. Eligibility Criteria The lead organization is eligible to apply. The program benefits children, pregnant women, senior citizens, chronically ill, and/or people with respiratory/cardiopulmonary illnesses and impairments. The program is designed to reduce the risks associated with asthma and other respiratory and/or cardiopulmonary ailments. The program serves populations within either the priority zone and/or eligibility zone.

11 EVALUATION CRITERIA 3. Additional Information
The applicant has provided a funding request between $500,000 and $1 million. The applicant has listed other sources of funding, if available. 4. Organization Description The applicant has described its organization such that the reviewer can understand the mission of the organization and services provided. 5. Proposal Narrative The applicant has proposed a project/program which uses evidence-based strategies capable of achieving primary and secondary objectives from the Community Health Guidelines. The applicant has described the roles and contributions of partner organizations if partners have already been identified. The program/project can be completed by December 1, 2020.
