Letter from Director Dear Friends, This past year has been an active one for our organization. In addition to serving our clients, Cumberland HealthNET (CHN) was busy in 2013 with efforts to strengthen our organization which included moving into a new location. We spent the previous three years at the Cumberland County Health Department, about a mile away from our new location. Our old office served us well but we could not be more excited about our new space. The board has also been busy making sure our organization is operating as effectively and efficiently as possible by initiating improvement efforts including: a review and update of mission and by-laws, review and update of financial reporting, and development of an organizational plan. Cumberland HealthNET provided services to many uninsured residents of Cumberland County in As we move ahead, it is our full intention to continue our same commitment to serve, with a goal to reach even more residents than we have in the past. We can not do this without the continued support and collaborative efforts of our staff, providers and community partners. As director of CHN, I would like to personally thank everyone that is involved with the organization and I invite those that are not yet involved to join us in serving our community. Preparing this annual report has given CHN the opportunity to reflect on the many accomplishments of our organization in I hope that you will review the information we have offered in this annual report. Who Are We? Cumberland HealthNET
Cumberland HealthNET (CHN) is a collaborative network of community organizations working together to provide access to healthcare for the uninsured residents of Cumberland County. Our focus is primarily on individuals with diabetes, asthma, congestive heart failure, and hypertension. Our Mission: The mission of CHN is to create a coordinated system of care for the uninsured residents of Cumberland county, resulting in healthier residents with a better quality of life. What Do We Do? Care Management Medical Home Prescription Medications Mental Health Nutrition Education Life Skills CHN assists patients in solving health related issues by connecting them to appropriate community resources to service their needs At CHN, a registered nurse checks the patients’ blood pressure during the enrollment process. In 2013, approx. 63% of patients had an out of normal range blood pressure. Patients are educated on signs and symptoms monitoring. What Do We Do? FACT: Cumberland County has a higher rate of “population with poor or fair health” as compared to the overall North Carolina State percentile in 2013 CHN coordinates health care services to adults in Cumberland County who are diagnosed with a serious medical condition, do not have health insurance, and have income that is at or below the federal poverty line (FPL). 2013 Federal Poverty Guidelines FACT: Cumberland County Death rates exceed the NC state death rates for all 5 leading causes of death. CHN enrolled 683 uninsured Cumberland County residents
How Do We Impact Our Community? CHN enrolled 683 uninsured Cumberland County residents in FY 2013 CHN partnered with Walgreens Pharmacy to offer three flu shot clinics in Fayetteville in 2013 50 patients received eye exams and eyeglasses for a copayment of $25 in FY 2013 Cumberland HealthNET “Navigating healthcare for the uninsured.” 2013 Annual Report

How Can You Help? Cumberland HealthNET
Physicians partner with Cumberland HealthNET to form a volunteer provider network to address the needs of the uninsured. Medical Homes who accepted CCH Patients in 2013: Interested in becoming a provider or offer help to CHN?
