Michigan Veterans Affairs Agency Notes: ***A DD214 can be requested by Veteran, next of kin, or surviving spouse. When the Veteran is deceased, the request must be accompanied by a copy of the death certificate. If the Veteran is alive and the signature is not the Veteran’s, the request must be accompanied by a POA.*** Was the Veteran’s home of record Michigan? If not, refer them to that state. Is the Veteran Air or Army NG? If so, direct them to (Air ) (Army ) Have they had possession prior? If so, did they use to apply for benefits at the VA or for a membership like the AL or VFW? They might have a copy.

DD214 Request Process Call 800-MICH-VET with Veteran’s Name, SSN & DOB
If we have it on file, mail/fax/ request form to MVAA Currently, MVAA can not fax or DD214s If we do not have it on file, send SF180 to NPRC NGB 22’s – Call Notes: ***A DD214 can be requested by Veteran, next of kin, or surviving spouse.
