Case study May Mrs P is referred in by the Falls Team as she has been identified as needing support with claiming benefits, some physical activity and transport. 3 separate partner referrals are generated to cover each of the identified issues. 2 to Bassetlaw Action Centre and 1 to Bassetlaw District Council.

BDC Active Communities contact Mrs P and arrange for her to attend a seated exercise class at Retford Leisure centre. BAC transport contacts Mrs P and a minibus leaflet is sent out BAC benefits calls Mrs P – speaks to her daughter and an assessment is completed. A call is also made to DWP for them to send out an attendance allowance form.

Mrs Ps daughter confirms she will contact BAC for help completing the AA form when it arrives.
