ILS tasks: Recruit eligible consumers as participants; Implement HAIL steps with consumers; Record how HAIL steps are implemented with each consumer using an existing system or the HAIL tracking form; Communicate with the HAIL researchers about program implementation.

Data Collection with ILS
During HAIL Implementation: Record how the ILS worked with each consumer: How much time does the ILS spend working with the consumer? What’s the format of the communication (e.g., in-person meeting, phone conversation, text, ) between the ILS and the consumer? What are the main activities occurred during each contact? How often does the ILS communicate with the consumer? We can either ask the ILS to fill out a form or extract this information from their existing database if applicable.

During HAIL Implementation: Assessment of goal achievement with the Goal Attainment Scale (GAS) Purpose: to measure the extent to which goals have been achieved 5-point measurement scale 1 = much less than expected outcome 2 = somewhat less than expected outcome 3 = expected outcome 4 = higher than expected outcome 5 = much higher than expected outcome

Goal setting (ILS and consumers) The ILS assist consumers to set short-term SMART goals 1 = much less than expected outcome Current level of each goal 2 = somewhat less than expected outcome 3 = expected outcome Identified short-term SMART goals 4 = higher than expected outcome 5 = much higher than expected outcome

Assessment of goal achievement (cont.) Goal measurement Researchers will define the other levels of goals using the GAS for consumers Researchers will confirm with the consumers and ILS Researchers will ask the consumers to rate their goal performance at the end of a goal pursuit period Researchers will ask the ILS to evaluate the goal performance of consumers for reliability

During HAIL Implementation: Record of consumers’ short-term goals What are the consumer’s short-term goal(s) for consumer skills? What are the consumer’s short-term goal(s) for addressing secondary health conditions? How is the consumer going to track his/her progress toward goals? What is the progress made upon each contact between the ILS and the consumer? What are the obstacles or challenges faced by the consumer?

After HAIL Implementation Researchers will interview (in-person or phone) the ILS about their experiences in using the HAIL program: What worked? What did not work? How to improve HAIL?
