All initial paperwork (overnight permission, behavior policy, code of conduct, intent)
Medical Release form – due by end of February
Copy of insurance card (front and back) – with Medical Release Form or by end of February
Rooming Lists – will be out by Monday, due before February break
Over the Counter, Prescription, etc. forms due to Nurse by end of February (prescription meds in original container due to Nurse by the week of April 10th)
Lunch Choices – Mid February (Google doc)
