Original Pathway for Patients Requiring Cataract Surgery
Patient referred by GP / optician Seen by consultant in the eye clinic and listed for cataract surgery if appropriate Return for a pre-operative assessment [POA] with the nurse practitioner Surgery date given 3x hospital visits per patient

Consists of 8 new patient appointments

Why Change? To reduce the amount of visits to the eye clinic for patients Reduce costs to the patient and the hospital Improve patient experience – less stress Reduce burden on family members Decrease number of episodes patients dilated and suffer vision reduction Gives a more streamlined and efficient service

One Stop Pre-Operative Assessment Cataract Clinics
Patient referred by GP / optician Seen by the nurse practitioner for discussion and examination of the eye Seen by consultant and listed for surgery if appropriate on same day Surgery date given 2x hospital visits per patient

One Stop Pre-Operative Assessment Clinic Template
Consists of 6 new patient appointments + 4 one stop patients alongside consultant regular clinic Capacity increased by 2 patients per clinic

Challenges Faced Staff reluctant to change practice
How to collect patient reported outcome measures with electronic patient record Need to change grading form to identify patients suitable for the one stop pre-operative assessment cataract clinics

Increased cost of patient transport to the HB Increased burden on patients and family to attend multiple hospital visits New Pathway Not all patients are suitable for this pathway

Patients will have more time to think of questions for nurse / consultant New Pathway Reduce visits to the hospital for patients Reduce travel costs for patients and Health Board Increase capacity and reduce waiting times Patients available to come at short notice for surgery
