Risk factors were defined as values at or above the age- and sex-specific 80th percentile for BMI, LDL cholesterol, systolic blood pressure or fasting insulin. For low HDL cholesterol, the age- and sex-specific 20th percentile cut-off point was used. Smoking was defined as a risk factor if the subject was smoking on a daily basis.
