Two broad approaches have been described: Multi-speciality Community Based Provider (MCP) - A Integrated Multidisciplinary Provider Primary and Acute Care System (PACS) - A Majority Provider for Local Hospital Providers

Population base approaches Aim to improve the physical, mental, social health and wellbeing of a defined population Base around General Practice and General Practice registered population Encourage Providers to work differently (collaboratively/partnerships) Holistic team base care Whole population based budgets Differences: Scale of included population - PACS being significantly larger - PACS offer transformation across the acute sector foot print
