2 History and physical examination
A 74-year-old man presented with long history of back pain and more recent (3 years) gradual onset of bilateral claudicant leg pain. He described his leg symptoms as somewhat radicular in nature , but more as a sensation of tiredness, heaviness and some numbness. The left leg is a little more symptomatic than the right. His leg symptoms had become more of a problem than his back pain because they limited his ability to walk more than 300 or 400 meters, or stand more than 5 or 10 minutes. He could not relieve his symptoms by standing, but rather had to sit down for a few minutes before he could start to walk again. He stood in a slightly forward flexed position, mild reduction in SLR test, average knee reflexes, absent ankle reflexes, booming ankle and foot pulse. Full range of movement in his hips.
