3 part Monitor Card Yellow section – To be filled out by Exiting Teacher Blue section – To be filled out by 1st Year Campus Green section – To be filled out by 2nd Year Campus DATE YOUR NAME

Exit Year Information The exiting campus and teacher should fill this portion of the card promptly after the End of year LPACs to place in the exit section of the blue folder.

1st Year Monitoring Every Six Weeks, this card should be updated with the students’ grades by printing from txGradebook and attaching to the back of the card. If a student happens to be failing a class the teacher of that class should provide intervention proof of what he or she is doing to help this student be successful. This can be hand written or typed and attached to the back of the monitor form. At the end of the year, these students shall be LPACed and discussed. Fill out the STAAR Mastery Section, make recommendations and the principal should sign this card for the appropriate year.

2nd Year Monitoring Same as 1st year Monitors. Every Six Weeks, this card should be updated with the students’ grades. No card? If a card was not filled out at the exiting campus, Start a Monitor card as soon as possible. Get the information from the PRC. If the student came to us from another district as a monitor, simply fill out the information that you can and then monitor the student’s grades. If the student comes to us as a 2nd year monitor, do not worry about filling out this student’s 1st year information, simply start the 2nd year monitoring.
