Problem Statement – 5 min: Our problem is the lack of process to review and assess the prescribed and over the counter medications for our patients. This problem impacts the workload of our interprofessional care team and the patients who attend our mood disorder clinic every 6-8 weeks. This problem is a safety concern because of the risk to patients from prescriptions not being filled, not being taken as directed or related to medication side-effects and interactions.
