Fatality Process At about 07:00, October O, OOOO, the victim went to the site and received work order from the factory superintendent. At about 08:00, the victim and a co-worker were doing grinding on outer surface of aluminum panels in the grinding workplace using hand grinders and brushes. At about 11:50, after finishing works for already received panels, 2 co-workers unloaded additional aluminum panels using a forklift on the grinding workplace. Fatality Process 3 co-workers loaded aluminum panels (1,170Kg) on the forklift on the right side of the victim. The forklift was moving forward without securing visual field to collide with the victim who was standing in front of the forklift. Fatality Cause Wrong forklift operating method To transport something using forklift forks using a forklift, you must drive the forklift backwards to secure visual field for driving direction when the front visual field is not secured. However, the operator did not secure the visual field. No division of safety passageway - You must secure clear path division between any forklift and worker to prevent forklifts from crushing into a worker. However, there was no clear path division. Precautions Improving forklift operation method When front visual field of a forklift operator is not secured owing to high loading, he must drive backwards to secure visual field in the driving direction. Added to that, you must improve forklift-operating method such as loading of only proper height to secure front visual field. Securing safety passageway and signs - You must secure clear path division between any forklift and worker to prevent forklifts from crushing into a worker and must secure safety passageway and division.
