SWIS School Wide Information System Completing and submitting is more important than ever. Will be used by state to monitor our program for suspension, restraints, and behavior overall. Every classroom must use!

What is SWIS Online database for tracking student behavior
Review NWGNETS behavior patterns: The five basic reports in SWIS frame the context within which problem behaviors occur in our classrooms helping the administrative team to answer these questions: How often do incidents occur? What problem behaviors occur most frequently in our network? Where are problem behaviors most likely to occur? How often are students physically restrained or suspended? Which students are involved in referrals?

When is a SWIS needed? Major Disciplinary Incidents:
Examples include: aggression, property destruction, leaving class/running from the group without permission, office or bus write up, excessive cursing of peers/staff, suicidal threats, etc. If a behavior incident requires administrative involvement or results in a drop in level and is not one of the above listed complete a SWIS. Anytime you feel documentation of a student-student or student-teacher interaction may be needed. Can be completed by teacher or paraprofessional, needs to be reviewed by teacher before submission to administration or parent.

Completing SWIS—review for relevance with new form
SWIS is completed and submitted to administration and parent within 24 hours of an incident Fill in all sections of the form completely, including grade, date, location, and time of offense If multiple incidents of behavior occur in one day you will have to complete more than one form You can only select one problem behavior on the form, it should be the most severe behavior displayed during the incident In the description do not use other students names. Keep the description clear, overt, and concise. No personal interpretation, no direct quotes, and no emotional language. This section should be BRIEF. Explain how we used to say list the antecedent but then it made it look like we gave severe consequences for less severe behaviors. We will use SWIS to send home to parents as written notification of a physical restraint. The separate physical restraint form is also completed within 24 hours and will be submitted to principal or designee and area admin.

Motivation, do not mark other or unknown. When in doubt put obtain adult attention. There are often several consequences for a major behavior, check the one box for the most severe administrative decision any others can be listed as part of the reflection/action plan. If a student is suspended the number of days MUST match what the school office reports If restraint was used, you MUST complete a separate physical restraint form. You can cut and paste as appropriate to save time SWIS should be signed by the student if appropriate and the adult that reviews their consequences and action plan with them Give an example of how others can be listed in the reflection

Reflection and Action Plan This is probably one of the most important sections on the form and most often overlooked. List therapeutic interventions attempted to help prevent behavior and calm student that day Describe modifications to tried therapeutic interventions or new ideas to help prevent the behavior from reoccurring or to de-escalate in the future. Always print a copy for the student data notebook!!
