Signing onto CMS Healthcare Personnel Safety Module Reporting Influenza Immunizations Among Staff

3. For those already enrolled in NHSN, log into NHSN and
click on “NHSN Reporting

4. When you get to this “Landing Page” (your screen may appear differently
than my log on) click as you would do to advance to the next screen.

5. The next screen to appear should be your “Home Page”
The next screen to appear should be your “Home Page”. Click on “Facility”

6. Select “Facility Info”
Select “Facility Info”. When the “Edit Facility Information” screen appears,
scroll down….(next slide)

7. When the “Edit Facility Information” screen appears, scroll down to
“Components Followed” - Check the box next to “Healthcare Personnel
Safety” and scroll down to bottom of the page and click on “update” to
save changes.

Go up to internet address bar and type in When the CDC screen appears, click
On the CDC A-Z Index in the upper right corner of the screen. A blue bar with the alphabet will come
up and click on the letter “N”. Scroll down and select the National Healthcare Safety Network (NHSN).
Select “Reporting & Surveillance Resources for Enrolled Facilities” and click on “Ambulatory Surgical
Centers”. Then open up the link in the red box bellow>
