RAMP 90-Day Aim for Action Period 2: By May 24, 2016, 90% of NHVs will know the status of referrals to EI services within 30 days of referral to EI Services. Change being tested: Using a tracker sheet to know the status of EI referrals Test #1 Test #2 Test #3 Test #4 What: 2 NHVs using a standardized process (tracker tool) to follow referrals to early intervention services (Head Start) Who (population): Heidi Kasey When: 2/26-2/26 Prediction: -Both nurses (100%) will know the status of their referrals to EI services Results: Both nurses were able to track status of referrals, but found that the form used for tracking was not user friendly Act: Adapt form Adapt: Abandon: Adopt: What: No new nurses added, revised the form to see if tracking would be easier 3/15-3/22 Tracking form will be easier to read and less time to track data. 100% of nurses will know status of referrals and time tracking will decrease by 1 hour. Results: Form was easier to read but we had excluded some of the information needed for data collection. No change in amount of time spent. 100% of nurses knew status of their referrals. Act: Adapt: Form revised again Form revised for 3rd time, 2 new nurses added Kelly Donya 3/22 -3/29 Form easier to use, tracking time about the same due to roll out to new nurses. 50% of nurses knew referral status (Kasey & Heidi ) Act: Adopt, form rolled out to 2 more nurses 2 new nurses added to process Michelle Amber 4/4-4/8 100% of nurses will know status of referrals and time tracking will decrease by 1 hour
