6 Opting Out Step 1 Go to: Step 2 Fill in online forms* Step 3 Collect opt-out cheque from Health Benefits Office (dates TBA) *Must provide proof of alternative coverage (i.e., policy and group number for current benefits provider)

7 Opting In Step 1 Go to: Step 2 Complete opt-in form Step 3 Bring form, required documentation and cheque* to Health Benefits Office *Please make all cheques payable to the Trent Graduate Students’ Association
