How the Collaborative works
The term Collaborative is a framework for improvement The Collaborative team work with Community members in partnership The community members focus on Health and Social care issues within their own community We support them to gain insight plan and deliver real change in their communities

The Collaboratives story
2007 – two projects: Older peoples/cancer 2009 – Coronary Heart Disease 2010 – Skin health (reducing pressure sores) 2010 – lung cancer 2011 – laryngeal cancer 2011 – cancer champions training (funded by Macmillan) 2012 – Winter planning 2012 – Ovarian and Oesophageal cancer 2013 – Re-branding as Health & Wellbeing 2013 – Kidney and bladder cancer
