3 Confined Space Entry Permit
What is a confined space entry permit? The written or printed document that is provided by the employer who designated the space a permit space to allow and control entry into a permit space and that contains the information specified in 29 CFR

5 Permit Space Information
Introduction The permit space to be entered. The purpose of the entry. The date and authorized duration of the entry.

6 Hazards and Control Measures
Introduction The hazards of the permit space to be entered. The measures used to isolate the permit space and to eliminate or control permit space hazards prior to entry.

7 Communication and Rescue System
Introduction The communication procedures used by authorized entrants and attendants to maintain contact during the entry. Any additional permits, such as for hot work, that have been issued to authorize work in the permit space. The rescue and emergency services that can be summoned and the means (such as the equipment to use and the numbers to call) for summoning those services.

8 Special Equipment and PPE
Introduction Equipment, such as personal protective equipment, testing equipment, communication equipment, alarm systems, and rescue equipment, to be provided to authorized entrants. Means of detecting an increase in atmospheric hazard levels in the event the ventilation system stops working.

9 Roster of Authorized Personnel
Introduction The authorized entrants, by name, within the permit space. Each person, by name, serving as an attendant. The individual, by name, serving as the entry supervisor, and the signature or initials of each entry supervisor who authorizes entry.

10 Results of Tests and Monitoring
Introduction The results of the tests and monitoring performed, accompanied by the names or initials of the testers and by an indication of when the tests were performed.

11 Additional Information
Introduction Any other information as necessary, given the circumstances of the particular confined space, to ensure employee safety.

12 The Permitting Process
Introduction Each entry employer must complete an entry permit prior to entry. Entry supervisor must sign completed entry permit prior to entry. Entry permit must be posted at the entry portal. The duration of the permit may not exceed the time required to complete the assigned task.

13 The Permitting Process
Introduction The entry supervisor must terminate entry and take the following action when any of the following apply: Cancel the entry permit when the entry operations covered by the entry permit have been competed; or Suspend or cancel the entry permit and fully reassess the space before allowing reentry when a condition that is not allowed under the entry permit arises in or near the permit space that is temporary in nature and does not change the configuration of the space or create any new hazards within the space; or Cancel the entry permit when a condition that is not allowed under the entry permit arises in or near the permit space and that condition is not covered in the above paragraph.

14 Permits must be posted at the entry of the permit space

15 Records Retention Introduction The entry employer must retain each canceled entry permit for at least one year to allow for the required review of the permit-required confined space program. Any problems encountered during an entry operation must be noted on the pertinent permit in section 13 so that appropriate revisions to the permit space program can be made.
