Data Driven Quality Improvement: Tools to Identify and Apply the Best Countermeasure to Facilitate Successful Change Grant M. Greenberg MD, MA, MHSA Jennifer L. Johnson, MSHS Michael D. Mendoza, MD, MPH, MS
2
Disclosures None of the typical kind but…
We like to lead change and improvement
3
Structured Problem Solving
5
Project Timeline Example
“If I had an hour to solve a problem, I would spend 55 minutes thinking about the problem and 5 minutes thinking about the solution.” --Albert Einstein
6
The problem with problem solving: Our Natural Human Tendency
7
3 common problems in problem solving
Assuming you know what the problem is without seeing what is actually happening. Assuming you know how to fix a problem without finding out what is causing it. Assuming you know what is causing the problem without confirming it. In other words - not grasping the situation.
9
Think/Pair/Share Reflect on a process improvement effort that you led or participated in that did not work: List the reasons it did not work
10
Problem Solving Technique: 5 Whys
Ask Why x 5 (deceivingly simple)
11
Problem Solving Technique: Fishbone
13
Problem Solving Technique: Flow Charting
14
Think/Pair/Share Using the same example you used before, briefly diagram the root causes using either: 5 whys Fishbone Flow Charting
15
Finding a Problem is Necessary, but not Sufficient
Should you fix it? What is the anticipated benefit? How hard will it be to do so? Do you have adequate time, resources? Is the problem in an area under your management?
16
Pareto Chart Technique to determine most significant cause of a problem
18
Final Thoughts What happens if you solve the wrong problem? What if it is not clear where responsibility lays for a particular problem? What is your role in problem solving?
