Sequence of events Marine OBC vessel working offshore Tanzania
Starboard engine failure causing fumes/smoke to fill engine room at night. Fumes/smoke flow through vent out of engine room. Starboard engine stopped immediately Sleeping container near engine room vent with A/C unit running drawing fumes /smoke into sleeping container where 8 workers were sleeping. Navigator on duty alerted sleeping workers. Workers evacuate sleeping container. Sleeping container ventilated. Sequence of events 8. Contractor ordered immediate stand down of project. 9. All vessels return to port for repairs and medical evaluation of workers involved. 10. Investigation analysis
The 8 workers that were in sleeping container complained of headaches and some breathing problems. Sleeping container did not have smoke alarms. All workers from sleeping container were checked by Physician. No medical issues noted, all cleared to return to work. Contractor requested complete vessel audit by qualified 3rd party be conducted prior to vessel being allowed to return to project. All action points from audit addressed prior to return to project.
