3 Project aim To develop a general/family practice RefSet of SNOMED CT, containing SNOMED CT concepts frequently used by GPs/FPs internationally. The RefSet will then be mapped to the International Classification of Primary Care, Version 2 (ICPC-2)

Phase 1: Development of project framework (months 1-9) Phase 2: Construction of the GP/FP RefSet and map from the GP/FP RefSet to ICPC-2 (months 9-15) Phase 3: Field testing the GP/FP RefSet and map from the RefSet to ICPC-2 (months 16-21)

5 Phase 1 – Development of project framework
Define methodology and tooling for the construction of the GP/FP RefSet and ICPC-2 map 9 month duration – due for completion May 2011 SNOMED and ICPC are complimentary as they fulfil different functions in an electronic health record and in analysis and reporting functions. The way in which terminologies are implemented in primary care IT systems greatly influences the quality of data for both clinical and secondary purposes. Primary care physicians are “creatures of habit” in regard to terminology use and exhibit high levels of variance in use of terms. Terminology systems must adapt this variance rather than try to eliminate it. Primary care physicians frequently have a strong public health role in their communities and this must be reflected in the way data is presented and analysed.

7 Main deliverables – Phase 1
Scoping document – a document that defines the scope of the RefSet, including an evaluation of the data elements used in general/family practice Requirements document – a document containing requirements for the general/family practice RefSet and map to ICPC-2. The document will also contain requirements for governance, tooling, criteria for testing and approval of the RefSet and ICPC map and requirements for maintenance. Design document – a document that contains the technical and technical/structural design elements of the general/family practice RefSet and map to ICPC-2.

8 Phase 2 deliverables The international GP/FP SNOMED CT RefSet (including RefSets for reason for encounter and health issues) The map from the GP/FP RefSet to ICPC-2 Recommendations for individual country extensions of SNOMED CT Recommendations for new concepts to be added to the SNOMED CT core

9 Phase 2 tooling For mapping the source general/family practice termsets to SNOMED CT: CSIRO Snapper mapping tool For mapping the GP/FP RefSet to ICPC-2 ICD-10 prototype mapping tool (with ICPC-2 loaded and suitable modifications made if needed)

Report from field testing Recommendations arising from field testing Final GP/FP RefSet ready for implementation Final map from the GP/FP RefSet to ICPC-2 ready for implementation Guidelines for implementation of the RefSet and map Final project report
