3. Student Referral and Assessment Program (LCMHSADS/LCPS Collaborative Program)
Student with difficulties due to:
behavioral problems
emotional problems
family issues
peer relationships
other student adjustment challenges
in need of an assessment
Parent written consent is required to start the process.
School Counselor makes the referral to appropriate LCMHSADS clinician

4. Student Referral and Assessment Program
Protected by Health Insurance Portability and Accountability Act (HIPAA) policies and procedures
Is subject to the Family Educational Rights and Privacy Act (FERPA)
UP TO 3 student visits
Parent consultation
Letter with recommendations and, if appropriate, referrals (typically for a private provider that accepts family’s insurance)

12. Crisis Intervention Least Restrictive Environment
Telephone support to assist in identifying natural supports and/or alternative responses to impending crisis, speak with supports. Have them follow up with their individual therapist, if applicable.
In CITAC or Emergency ROOM: Support individual and support people in developing a Crisis Action Plan and Crisis Support Plan. CR
