Online View and make any changes to your Frontier health & welfare benefits at: Frontier Benefits Center Don’t forget to review your beneficiaries and print a copy of your elections User ID: first 2 letters of your first name, first 3 letters of your last name (all lower case) and last 4 numbers of your Social Security Number (SSN) Password: If you are logging for the 1st time, your date of birth (MMDDYYYY). Review your health care Benefits for the upcoming year Changing your Benefits elections during the year Your current medical, dental, Vision, life insurance, AD&D insurance will automatically continue for 2018 Your current Flexible Spending Accounts election will NOT continue for 2018; you must re-enroll during this Window To change your coverage and/or enroll for Flexible Spending Accounts, you must make an affirmative election as part of Annual Enrollment Annual Enrollment is generally the only time during the year that you can make changes to your coverage, unless you have a qualified life event (such as birth of child). For information and what constitutes a qualified life event, refer to your Summary Plan Description (SPD) available in the Resources section of the Frontier Benefits Center. It is your responsibility to initiate the qualified life event within the 31 days of the effective date of the event: OR Complete/Update your beneficiary statement online It is important to verify your covered dependent(s) information, enrolling ineligible dependents violates the terms of your benefits plans If you have a qualified life event (such as birth of child) between now and the end of the year, you will need to make any necessary changes on Frontier Benefits Center for both 2017 and 2018 Save/print a copy of your confirmation statement, confirmation statements will not being mailed.

Medical Plan until age 26, no other requirements applies
Dental & Vision Child Life and Child AD&D Plans end of the year in which reaches age 23, no other requirements applies Eligible Dependents Your Frontier Health & Welfare plan can cover your: Spouse Same-sex Domestic Partner Children and eligible children of your Same-sex Domestic Partner You are responsible for updating your Child Life Insurance and AD&D insurance elections if your previously eligible child(ren) no longer meet the eligibility requirements as noted above. Your Insurance Company will not honor the ineligible child’s claim. If you add any new dependents during open enrollment, verification is required: Frontier Benefits Service Center will mail a dependent verification request upon adding a new dependent to medical, dental and/or vision plans You must provide the required documentation by the due date indicated on the form Newly added dependent(s) will have coverage once their eligibility is verified

What’s New for 2018 Medical Plans - are offered based on the terms of your collective bargaining agreement Employee contribution - for the medical plans are adjusted based on the terms of your collective bargaining agreement New Prescription Drugs Administrator – Express Scripts (ESI) will be the prescription Drugs administrator for 2018(view slides 8-11 for details) CarePlus Plan - is eliminated effective November 1, 2017 based on the terms of your collective bargaining agreement Health Care Flexible Spending - annual maximum will increase to $2,600 from prior year of $2,550 Employee Supplemental life and supplemental long term disability rates will be reduced

New Prescription Drugs Administrator – Express Scripts (ESI)
ESI will be the new pharmacy administrator effective 1/1/ As a leader in the pharmacy benefits management industry, ESI provides convenient, user-friendly services including: 24/7 customer service availability to members and pharmacists Pharmacists that are available around the clock for medication consultation Enhanced mail order delivery, including extended payment program Mail order cost can be spread over three credit or debit card installments More than 60,000 retail pharmacies nationwide; approximately 36,000 chain and 24,000 independents Members will receive welcome package prior to 1/1/2018 including ESI prescription ID Card and useful information on how to get started using mail order. Note: Plan design will be replicated based on the terms of the collective bargaining agreement

Transitioning to ESI- What you need to know
Current Mail Order User Open refills from CVS-Caremark mail order pharmacy will transition to ESI beginning mid-December, and will continue to transition to ESI through second week of January 2018. CVS-Caremark Mail Order pharmacy will continue to fill all refills requested through 12/31/2017 New Mail Order Prescription Eff 1/1/2018 Effective 1/1/2018, you may either use the ESI Mail Order pharmacy or CVS retail stores to fill your 90-day mail order prescriptions. Do not send new prescriptions to ESI mail order pharmacy prior to 1/1/2018.

They are the same high quality as those you currently use Special handling of temperature-sensitive medications ESI takes care to protect the potency and effectiveness of your medications. Therefore, any medications that are sensitive to extreme heat or cold are shipped in special insulated packaging with gel packs designed to keep the contents at the correct temperature throughout the delivery process. In addition, you can arrange for these types of medications to be delivered at a time and location that’s convenient for you, thus helping to keep exposure to heat or cold to a minimum. Getting started at ESI By Mail Ask your doctor to write a new prescription for your plan’s maximum days’ supply (usually 90 days), with refills for up to 1 year, as appropriate. Mail your prescriptions in the special envelope that you receive with your enrollment materials or ask your doctor to call for instructions on how to fax them. If your order is faxed, your doctor must have your member ID number to complete the transaction (Only your doctor can fax prescriptions to ESI).

Part-time employees working less than 25 hours p/week can view their per/pay contribution online during the AE Window: Working between 17 – hours p/week will pay 50% of the premium Working between 0 – hours p/week will pay 100% of the premium

Flexible Spending Accounts – Annual Maximum Contribution
Health Care Flexible Spending Account annual maximum contribution is Increasing from $2,550 to $2,600 in 2018. The FSAs Summary Plan Description at provides specific information about the FSA plans and eligible expenses Health Reimbursement Account (HRA) – If you have an account balance, be sure to submit claims to get reimbursed for the eligible out of pocket expenses. Important – If you enroll for the Healthcare FSA and have HRA account balance, you are required to utilize your FSA account balance before you can use your HRA account balance.
