Evidence-Based Practice
3
The Process
4
The Process Select a topic based on an existing problem, new knowledge or opportunity to impact patient experience. Form a team with key stakeholders. Retrieve evidence by identifying sources, key terms and assistance from librarian and CNS. Grade the evidence based on source, type and criteria from table. Develop EBP standard by setting recommendation for practice. Implement EBP by participating in writing policy. Evaluation change through feedback and NDNQI data as applicable.
5
Where do you find good evidence?
Cochrane Collaboration CINAHL Ovid Up-to-date
6
What is good evidence? Accurate Precision Sufficient Representative
Authority/Credibility Clarity of presentation of results Recent
7
Evaluation of the evidence
Source of evidence Peer Review Number of participants Significance of findings Transferability
10
Levels of Evidence Systematic Reviews Clinical Practice Guidelines
Compilation of best available evidence Clinical Practice Guidelines Best available evidence and expert opinion to guide practice Randomized control trials Intervention vs. control group with randomization Cohort studies Monitor group of patients with existing condition over time Case Studies Monitor group of patients with condition vs. those without Expert Opinion
11
Developing a question Population Intervention Comparison Outcome
Example: For persons entering a health care facility, is use of waterless, alcohol-based sanitizer as effective as washing with antiseptic soap for reducing hand contamination?
12
Implementation Evaluate current practice Identify barriers
Communicate with staff about upcoming changes Evaluate resources
13
Your responsibility Pose a clinical question
Be knowledgeable about current practice Utilize librarian and facilitator for research assistance Read research Make clinical practice change recommendations with assistance from facilitator Three month commitment

Final Thought “The secret to change is to focus all of your energy, not on fighting the old, but on building the new.” Socrates
