Authorization § Authorizes Commission to assess fines and penalties for failure to file required reports $100

Review and Abatement R 67-1401
Request abatement in writing within 30 days Include reasons for contesting assessment File with department issuing assessment Department director will review May abate for good cause Determination set forth in writing Party may request Order and Rule to Show Cause hearing

Informal Conference R 67-804 (H)
Fine assessed for not appearing at an Informal Conference $188.36

Form 12-A R Due within ten(10) business days of the employer’s representatives knowledge of the accident If accident meets the criteria to file with the Commission
