INCIDENT: On 23rd July 2010 at about 7.40 AM, Birba (Transport subcontractor of SGEE) HD driver was descending down from the prime mover cabin after completing the inspection of the prime mover when, he slipped from the second step of the prime mover cabin steps and had a heavy fall on his left shoulder that resulted in sustained a fracture to the midshaft (left) humerus “U” slab (main bone in between the left shoulder and elbow) CAUSES: Lack of attention and due care while performing a routine task (getting down from the prime mover using originally provided 3 steps) LEARNINGS: Accidents can happen while performing most routine tasks. Recommended Actions: Discuss this alert in toolbox talks and HSE meetings and Distribute to all contractors Post on HSE notice boards Include in site HSE induction Look at the steps and your footing carefully while descending from prime mover.
