2 ACTIVITY What is your local service network for clients with decreasing independence look like?
INSTRUCTIONS By forming groups of people working in the same community, describe your local service network for clients with decreasing independence on the sheet provided. Please write the name of your community on top. In determining ways to plan and deliver a well-organized menu of services for a specific clientele, local communities must first identify and understand their available resources and needs. Key to this process is the idea that resources refer not only to fiscal capacity but also include people’s knowledge, skills, and personal and professional relationships. Community resource mapping is a process that can be and should be tailored to respond to the specific challenges, strengths, and context of any community. Mapping of services, supports, and programs within a community can have essentially three outcomes: the identification of resources available to targeted clientele in a particular community the identification of new or additional resources to sustain existing specific activities or initiatives within a community for a targeted clientele , and/or the identification of resources to assist in creating and building capacity to support a more comprehensive community system for serving the targeted clientele. The first outcome typically occurs at the local level while the second and third outcome can happen at any level—local, regional, national.
