In the management assessment report, an OFI was recommended to prevent a similar incident from occurring

Why integrate LL and issues? The majority of shared Lessons Learned come from issues we have entered into the Issues Management System (caWeb) Reviewing OPEX articles may provide an issue to enter into caWeb to take preventive action When it is uncertain how widespread an issue might be, we use an LL to share information with others to help address the potential Extent of Condition

Performing a Vulnerability Analysis on OPEX is an option at NSTec U.S. DOE Office of Environmental Management, WIPP Accident Investigation Report (AIR) distributed throughout the NVE U1a Complex performed a vulnerability analysis based on the WIPP AIR Citations of identified weakness were extracted from the accident investigation report Analyst compared NSTec’s Process and data to the identified weakness Risks or Program and Performance Vulnerabilities were identified Reference to existing issues or new issues were created to track mitigation strategies

Integrating Procurement with OPEX
OPEXShare has recall information on products. We can contact Procurement to perform a search on items at NSTec and take them out of service if NSTec has purchased any of them.
