2 Why is Monitoring and Evaluation Important?
Service Driven & Needs Led Person Centred & Outcomes Focused Self-directed support is about change. Service providers will work in new ways, and perhaps develop new processes, procedures and “cultures”. Service users and families will aim to achieve goals as well as meet needs.
