4. Entering a Contact Report
In Commissioner Tools, navigate to the Unit you wish to do a report on
Select “New Contact “
Select “Type of Contact”- Unit Meeting default -asterisked fields required
Select “Date of Contact” (Can load a date in the past)
Select “Unit health” – choose “declined, improved or unchanged”
Select either:
“Detailed Assessment”- twice a year
“Simple Assessment” – monthly or regularly
Simple Assessment - Fast and simple way to record a unit contact
Record concerns and/or positive ideas discussed
Complete an “Assessment” with every contact
Value of Assessment – measurement & feedback
“Performance Improves When Performance is Measured and Feedback Given”

6. Completing a Simple Assessment
Select “Overall Assessment Score”
1-5 in half point increments
Click on “Score” you desire to give the unit
Select “Comments Box”
Enter information on how service plan is being implemented

Other relevant information
Select “Save and Complete Contact”
This completes a Simple Assessment
System will return to Unit Screen
Notice Health Score in Upper Right Hand Corner
