3. New model from 1 April 2018: key features
A Doncaster specific model
Individualised care, rather than face to face or non face to face pathways- including psychosocial support for e-cigarette users
Targeted priority groups
A somewhat simplified performance related payment based on ‘routine and manual’ validated quits (10% of contract value)

4. New model- the targeted groups:
Routine and manual workers
Mental health clients
Prisoners on release
Chronic disease/complex health needs/pre/post operative
Reducing inequalities between communities
Targeting these groups will require the development of pathways into the service

5. Exclusion criteria Children and young people (under 18)
Pregnancy/and significant others during pregnancy and 1 year post natal
Hospital inpatients/prisoners in custody

6. Key outcomes/contract aims
To reduce prevalence of smoking in targeted groups in Doncaster
To contribute to a reduction in smoking related mortality in Doncaster

7. The Contract 4 years plus 2 12 month extensions possible
