Identifying and engaging our Community Champion with lived experience! Incorporating this concept into various programs; e.g. My Heart Program Clarence Hill

The importance and unique value of including a community champion with lived experience in our efforts; from conceptualization through implementation and evaluation.

What we learned about engaging community members with lived experience
Opportunity to obtain rich, qualitative feedback and guidance on our programs, initiatives and strategies. Enlightening, uplifting experience Provides a sense of purpose and renewal

What we learned about engaging the BROADER community in developing our aim
Encourages ownership in the process and success Development of meaningful partnerships is more likely (again, ownership) Anticipate will lead to improved outcomes

Our aim for SCALE is….. EXAMPLE: By Dec 2016, at least 25% of adults in our community will lessen their risk for future chronic diseases by engaging in physical activity opportunities, as measured by social media campaign challenge reach/activity. We plan to improve equity by implementing strategies to ensure minority populations are equally engaged in physical activity opportunities and the social media campaign challenge as the population as a whole.

What we learned from “Switch”
Rider: importance of identifying bright spots for what is working well and how Elephant: help motivation by setting short-term, attainable goals (shrink the change) Path: easier to clear the path for the elephant (give smaller popcorn buckets)
